Welcome to this AOBB Onboarding page! If you have access to this page, that means you are a great candidate for the applied position. Please complete your “New Hire OnBoarding Package 2” below and return ASAP. Here you will find documentation for the following:

New Hire Check List

  • New Hire Check List

  • Form W-2 (Office Employees, only)

  • Form 1-9

  • Copy of Social Security Card and Driver’s License (can email a clear copy of both)

  • Consent forms

  • Employee Confidentiality Statement

  • Hepatitis B Statement

  • Authorization for Release of Information

  • HIPAA Agreement

  • Client Rights Statement

  • Cultural Diversity Information

  • Introduction To Intellectual Development Disabilities Manual and Exam

  • AOBB Core Value Handout and Exam

  • Handbook with acknowledgment signature page

Other Documents Needed:

  • Copy of highest diploma/degree (i.e., GED, High School, Community College, University/college, Masters, etc.)

  • All up-to-date Training Verification required according to applied position, (i.e., NCI, CPR, First Aid, certificates, etc.)

  • Current Insurance Policy/Coverage

  • Home Insurance Policy/Coverage (AFL providers only)

  • Depending on the position applied for other documentation and training may be required

  • AOBB does offer training for positions if is needed or if certain certification has expired for DCS workers and office staff.

Acrobat Reader DC is best to use for signing all documents while employed at AOBB, it’s free to fill and sign documents. This program can be used on cellphone, tables, desk tops, and iPads.
